(1) New Myanmar Passport Application Form
(2) Your duly filled Personal Informatin Form

Download 7th Page of the Form https://servimg.com/view/11153621/23
(3) Your Icome-tax Paid certified Vouncher or Letter which would cover your New Passport applied date.
New Myanmar Passport Issuing Fee :
13833 Rupees ( New Myanmar Passport : 13500 Rupees + Form : 333 Rupees )
